I give my consent to be videotaped during my online sessions for the purpose of my provider being able to review her work with me as necessary, and potentially in professional consultation, in order to provide me with the best possible care. I understand that this opportunity for additional time and expertise focused on our work together is offered at no additional cost to me.

Any videotaped material will be held to the standards of confidentiality as outlined in the Client Agreement, and may not be used in any other way without my express written permission. Sessions will be erased after the therapist has completed their active review. My personal identifying information will not be disclosed or attached to any recording, and I may choose – at the conclusion of any videotaped session – to have the session immediately erased and not used for any purposes. This consent will be in effect until revoked.

I understand that I am under no obligation to consent to being recorded, and that my provider will provide me with the best possible care regardless of my choice in this matter.
